SERVICE COORDINATION

Service Coordination helps people with developmental disabilities get the information they need to understand their options and make informed choices. Individuals and their families are supported in accessing community resources, making transitions and selecting appropriate services. Service Coordination also assists with exploring options such as Social Security, other county benefits, L.E.A.P. and food stamps.

 

Every individual eligible for Horizons services has a Service Coordinator to assist in accessing services and supports. Service Coordinators monitor services, act as liaisons with other service providers and assist in the development of a plan for services and supports.

ELIGIBILITY AND HIPAA

Effective August 1st 2013, the state of Colorado changed the definition of a developmental disability. To meet the criteria for services, an individual age five or older must show evidence of a cognitive or adaptive behavioral impairment at least two standard deviations below the mean.
